We are currently recruiting for a Mental Health Practitioner with experience in working with complex cases, carrying our risk assessments and provide Support to those affected by Mental Health.
An excellent opportunity available for a Mental Health Practitioner located in Cranbourne. This role involves working hard to develop and nurture each person's ability to have greater control over important life choices and to strengthen their hope and motivation for change and recovery. Provide flexible support to clients and their families/carers ensuring each client is given the best opportunity to achieve their recovery details.
This role will include:
- Undertaking initial mental state and risk assessment
- You will be required to support clients through the NDIS changes
- Long-term clinical case management of clients, maintaining appropriate documentation and outcome measures
- Developing appropriate treatment and management plans including crisis plans
- Participation in multidisciplinary team meetings and discussions
You will have:
- Flexibility, good team communication and good case notes are essential
- Tertiary qualification in Social Work
- Hold a Right to work in Australia
- Previous experience in a social work role, completing case work for complex clients
- Previous experience in Mental Health.
- Knowledge of social justice principles
You will also be willing to undergo police and Working with Children Checks, as well as providing 2 recent manageable references.
