About the Role:

Reporting to the Program Manager - Virtual Care, the Wellbeing Manager - Telehealth will provide face to face support to clients in the Central and East Sydney regions who participate in our Telehealth service for seniors over 70 years. The goal of the service is to help people build their knowledge and skills in self-management of their chronic disease, working closely with their GP and other supports, to assist them to be healthier, independent and socially connected.

The position is a mobile role with many visits to client's homes, GP surgeries and health partners each week, you will be the key clinician and face of this Telehealth initiative.

Your key responsibilities will include:

  • Visit our clients in their homes to conduct holistic assessments, and production of goal driven support and wellness plans, incorporating wellness based goals to achieve healthy, active, independent and socially connected lives
  • Provide chronic disease and health education to clients and their families that considers cultural, spiritual, intellectual and psychosocial needs, and preserves autonomy, dignity and rights to promote long term wellness and self-management
  • Installation, training and ongoing support to clients in the use of Telehealth and emerging technologies
  • Linking and supporting clients to the appropriate supports and services to optimise choice and positive health outcomes
  • Develop positive and productive partnerships with local GP practices and proactively network with local service providers and key referral agencies to promote Feros Care's Telehealth and care services
  • Act as an Ambassador for Feros Care, participating in community events and initiatives to build the Feros Care brand and promote our services

About You:

  • Experienced Registered Nurse with Current Australian Health Practitioner Regulation Agency (AHPRA) Registration
  • Demonstrated experience in assessment and goal orientated support planning within restorative care, rehabilitation and/or chronic disease management domains.
  • Demonstrated ability to develop robust and sustainable working relationships with GP's, referral agencies and health services that encourage cooperation, accountability and promotes Feros Care and it's services
  • Demonstrated skills in the use of smartphones, laptops, video conferencing and web access to systems for information, data entry and communication
  • Demonstrated ability to work autonomously with a high degree of drive, initiative, motivation and outcome orientation, with the capacity to deal with multiple and often conflicting priorities
  • Demonstrated knowledge and experience in contemporary chronic disease coaching, health literacy and case management
  • Well-developed organisation, negotiation, facilitation and communication skills
