EACH is looking to extend its highly successful Home Care Package program across the Inner East area of Melbourne.  The Case Manager position will be initially based in Lilydale.

The role of the Case Manager will be to plan and facilitate access to a range of services and support, including the administration of brokerage funds as required.  The Case Manager will be expected to work collaboratively with the Home Care recipient and their support network to monitor and review the older person’s care plan, ensuring it remains responsive to the person’s changing needs and circumstances

The position requires a highly motivated, well organised person, who can effectively engage individuals and community stakeholders, build positive and sustainable relationships, and demonstrate a person-centred approach.

The position requires initiative and an innovative, collaborative approach in the development of community inclusive opportunities in line with carer / care recipient consultation, with consideration of a dynamic and evolving sector and emerging service trends such as the Consumer Directed Care model.
