About the opportunity
This is an identified Aboriginal Position. Applicants must be of Aboriginal descent.
An exciting opportunity exists for a permanent part-time (20 hours) Entry-level Support Advisor to join our Home & Community Care team located in Broken Hill this is an Aboriginal targeted position. Reporting directly to the Senior Support Advisor, in partnership with our clients and their family, you will provide excellent case management to enable them to live with meaning, purpose and independence.
This role aims to improve outcomes for Aboriginal people by enabling them to access culturally appropriate community and in-home support services.
Responsibilities include:
• Client assessment, referral, review and goal planning
• Client funds management and optimization
• Development and negotiation of clients’ individual budgets
• Monitor and translate client feedback into improvement opportunities
• Relationship management with internal and external stakeholders
• Promote Uniting in the community to potential clients
• Commitment to a cohesive and harmonious team
Essential qualifications
• Current First Aid Certificate
• Current Australian Drivers Licence
• Qualifications in Aged Care or Community Services (desirable)
• Certification with the Case Management Society of Australia (desirable)
About you
Key to the success in this role is the demonstrated ability in the below skills and experience:
• Understanding of Aboriginal culture, community health and wellbeing
• Ability to work autonomously and in a team environment
• Enthusiasm for the Health and/or Community Aged Care Services
• Excellent time management & problem solving
• Experience using client/data management systems (desirable)
• Understanding and delivery of case management (desirable)
