The Role
 
Due to a high volume of roles, we are currently looking for experienced and passionate Age Care Case Managers. We work with a number of ethical not for profit organisations as well as private organisations across Sydney and the wider NSW region and are recruiting for a number of roles

The Successful Candidate:

• You will have had demonstrated experience as a Case Manager in the Age Care area and have a passion for building relationships and supporting vulnerable people.
• A tertiary qualification within Social Care, Nursing or Aged Care.
• Be comfortable with representing the organisation and developing business where there are opportunities.
• Be knowledgeable about home care packages
• Hold a current drivers license
