QPP is a peer-based community organisation that has been committed to improving the quality of life of all PLHIV across Queensland since 1989.

 

This position will work within QPP's Life+ program.  Life+ consists of a multi-disciplinary team of people who work together to provide the necessary care and support services to people living with HIV across Queensland.  The goal of the Life+ program is to promote the health and well-being of PLHIV in order to build an individual's capacity to self-manage their HIV.  Life+ program staff will work collaboratively to achieve this goal by implementing a number of initiatives that promote treatment adherence and retention in care and improve individual health outcomes. 

 

About the role

The primary purpose of the Stigma and Discrimination Case Manager is to provide time-limited advocacy and referral for those PLHIV who experience stigma, discrimination or other HIV related legal problems affecting their quality of life. The positon will assess individuals for legal/advocacy support and refer externally. The position will also provide client-centred support to PLHIV who are experiencing barriers, which negatively affect their ability to commence and/or maintain HIV treatment and care.  This will require working under a case and self-management framework to build tailored action plans addressing the social determinants of health that impact treatment uptake, adherence and retention in care.  Under this case management approach, client circumstances and needs will be assessed holistically and clients will be supported to identify and overcome barriers, and develop skills to effect change in a way that is respectful of what is important to them while also achieving improved health and wellbeing outcomes.  A key aspect of supporting the achievement of client goals will be establishing and maintaining effective clinical and community access pathways. 

 

Skills and experience

1.     Tertiary qualification in social work/community development/law and/or social/human/legal services with prior experience in case management including advocacy.

2.     Maintain exceptionally high standards in the protection and promotion of client confidentiality/privacy.

3.     Successful completion of a six (6) month minimum employment period (probation) in accordance with QPP's policies and procedures.

4.     Ability to obtain an Application for a Police Certificate Name Only through Queensland Police and provide to QPP within three (3) months of commencing employment (this is to be completed at the worker's own expense).

5.     Ability to travel intrastate/interstate as required.

6.     Possession of current provisional or open Australian Driver's Licence.

7.     Willingness to work outside normal business hours as required.
