About the role:

The Support Coordinator will provide coordination of supports to participants who have National Disability Insurance Scheme Support Plans. They will work in partnership with people with disabilities (participants) and their ‘circle of support’ to develop, implement and monitor actions, focused on building capacity to achieve identified outcomes.

About you:

You have a passion for working with participants to build their capacity to live the life they choose. You can effectively sort through the jumble of support options with participants to facilitate their choices and create a clear and precise plan of action. You will enjoy the challenge of effectively encouraging and guiding participants through the Coordination of Support process to ensure they are on track to achieve their goals.

Selection Criteria

What you will need; qualifications/experience/qualities:

  • Ability to identify participant’s strengths and utilise these effectively to achieve outcomes.
  • A solution focused perspective and ability to inspire others to be positive and resolve issues.
  • Experience in developing, implementing and monitoring participant outcomes.
  • Focus on the importance of building a participant’s capacity to have choice and control over their life.
  • Be resourceful and have a good knowledge of informal, mainstream and funded supports and facilities.
  • Excellent administration, written, organisational and time management skills.
  • Excellent interpersonal and communication skills including the ability to advise and liaise across all levels of Mai-Wel and external organisations.
  • Highly developed computer skills, competency in the Microsoft Office suite and database systems.
  • Current drivers licence.
