Ability Assist aims to increase the independence, skills and quality of life of our clients who include people with support needs.  We provide a range of services including in-home and community based support and a day program for people with support needs including people with a disability. We also provide residential housing for people with a disability and contingency and long term housing for young people who require care. 

We are looking for experienced people to be a Team member to work with clients who have complex care needs related to behaviour, health and development, living in our houses. You must be a good team player and be able to use a therapeutic based approach to assist clients to more positively manage their behaviour and to access the community safely. You will need to have experience and skill in supporting young people and adults with complex care needs and behaviours.

You will be responsible for:

1.         Provision of direct support to the resident and role model such as assisting with living skills, communicating and interacting with the resident, facilitating community inclusion, maintaining an appropriate physical environment, providing transportation, maintaining finances, following behaviour support plans and encouraging the resident in becoming an active citizen in their neighbourhood and local communities.

2.         Participate in Support planning and monitoring by providing input to the house supervisor for planning and developing individual goals and outcomes with resident, coordinating and participating in support network meetings, monitoring, documenting, and reporting progress toward meeting outcomes, and communicating with other service organisations.

3.         Participate in training, support and appraisal sessions.

4.        Maintaining homes, vehicles, and property assets ensuring the house is homely and environmentally supportive to the client.

5.        Maintaining health and safety for both the client and staff.

6.        Supporting and promoting client rights.

7.        Adhering to policies, procedures, and code of conduct.

8.       Complete all required documentation, and other office task as necessary

           Basic computer skills required

 

To be successful in this role you will have as a minimum:

.

  • Desirable Cert IV Disability or tertiary qualification in a health, welfare or social science discipline.
  • Preferably 3-5 years' experience
  • Flexibility and available for shift work as we provide 24 hour care.
  • An interest in working with families and carers to support and assist in the lives of people with a disability.
  • Demonstrated skill in supporting people with major behaviours of concern with in a positive and therapeutic frame work.
  • Good communication skills both written and verbal
  • Ability to work as a team member working towards a common goal.
  • A current driver's license is required.
  • A current WWC
