Communicare is proud to partner with Yorgum Aboriginal Corporation, to be the lead agency for the enhanced Family Support Network in the Fremantle Rockingham region. The FSN provides integrated and coordinated secondary family support services to improve outcomes for vulnerable children, young people and families and prevent the need for tertiary child protection intervention.

Family Support Networks are a partnership of community sector services and the Department of Communities which provide a common entry point to services and deliver earlier more targeted support to families.  FSN’s provide services for families affected by issues such as mental health, family and domestic violence, homelessness (or risk of), family conflict and parenting difficulties.

Two Family Support Facilitators – Intensive Case Management will be employed by Communicare. A further Family Support Facilitator – Intensive Case Management will be employed by Yorgum Aboriginal Corporation to work alongside Communicare’s staff as a co-located, integrated FSN team.

The Role

The Family Support Facilitator - Intensive Case Management (ICM) is responsible for the intake, case planning and management of direct referrals from the Department of Communities to the FSN. The Intensive Case Management service is aimed at families requiring intensive support to keep their children safely at home, with a particular priority for Aboriginal families and children.  In collaboration with Departmental staff and other service providers, the Family Support Facilitator- ICM provides engaging, persistent and practical case management, mostly in families’ homes.

Selection Criteria

To be considered for this role, you must be able to demonstrate:

  • Tertiary qualifications in Social Work, Psychology or other related area.
  • Strong understanding of Aboriginal culture, family dynamics and experience working directly with families living in complex circumstances, using a trauma-informed approach.
  • Demonstrated experience in and commitment to providing intensive individualised support to vulnerable families and children, particularly Aboriginal families.
  • Experience working with families experiencing complex issues to build safety and wellbeing for children.
  • Demonstrated experience in providing place-based support services, including working in partnership with other organisations for benefit of client families.
  • Demonstrated ability to maintain strong working relationships with relevant service providers, in a multi-agency and interdisciplinary context.
  • High level communication and relationship building skills.
  • Ability to work Monday to Friday between 6am and 9pm on rostered basis.
  • National Police Clearance, Working with Children Check and Driver’s Licence
