 About the Service

The Continuing and Coordinated Care Program is a brand new program and model of care that will provide Coordination of Care and Continuity of Care for persons in NSW experiencing AOD use issues in conjunction with other areas of complex support need. The Program will support people who are experiencing barriers in accessing or remaining connected to services because of substance use disorder and/or experience difficulties accessing substance use treatment due to other co-occurring needs. 

 The Role

The Care Coordinator is responsible for ensuring that those supported by the program receive the care they require and want through direct delivery of support, ensuring care provided by different organisations and services is coordinated and ensuring continuity of care when a person moves from receiving support from one program or service to another. This role will report to the Care Coordination Team Leader.

The key responsibilities of the role will include:

  • the provision of enhanced care coordination and wrap-around support services for people with complex support needs including a substance use disorder

  • Coordinating direct support for those accessing the program, including provision of general drug and alcohol support such as Motivational Interviewing and Relapse Prevention

  • Developing, promoting and implementing referral pathways and facilitating access to a range of community based health and non-health service providers

  • Being mobile, working with clients within the community and focusing on quality client care and sustainable client outcomes

To be successful in this role, you will need:

  • Demonstrated experience in coordinating care for persons with complex support needs (including in particular AOD support needs co-occurring with serious mental health issues and other issues such as homelessness, trauma, contact with the criminal justice system and family and cultural separation).

  • Demonstrated ability to undertake assessment including utilising standardised tools

  • Demonstrated ability to work therapeutically with clients (e.g. provide therapeutic (treatment) interventions)

  • Demonstrated ability to conduct self in a professional, ethical and non-judgemental manner.

  • A relevant tertiary qualification or VET sector qualification and significant ongoing experience

  • Knowledge of, and ability to access, a range of relevant community resources.

  • Demonstrated ability to work as part of a team and to work independently and unsupervised.

  • Willingness to work within the values and ethos of the St Vincent de Paul Society and NSW Support Services

  • Computer literacy skills including experience in computerised data collection.

  • A current, Drivers Licence.
