Summary of program and its objectives:
The Retirement and Ageing Division at the Brotherhood of St Laurence provides residential services, independent living accommodation for people on low incomes and home based services for older people and people with disabilities living in their homes.
Main responsibilities of the role:
The Case Manager position consults and works with clients, their carers, service providers as well as the Brotherhood Aged Care team to provide integrated high quality home based services in accordance with funding and legislative requirements.
To meet the selection criteria, candidates will require:
Essential
- relevant tertiary qualifications in Nursing (Division 1), Welfare, Occupational Therapy, Psychology, Disability, Psychiatric Nursing, Physiotherapy, Social Work or other related discipline.
- proven experience in person centred case management and a reablement approach in the aged care and/or disability sector including provision of support to people with dementia.
- significant experience in the development of person centred planning and the ability to demonstrate a clear understanding of case management, assessments, networking and advocacy
- strong interpersonal and communication skills
- well-developed time management and organisational skills
- demonstrated ability to work autonomously as well as collaboratively as a team member to achieve goals
- demonstrated knowledge and competency in Microsoft Office Programs including Word, Excel and Outlook
- current Victorian Drivers Licence.
- understanding of and empathy with the values and ideals of the Brotherhood
Desirable
- experience in using The Care Manager (TCM) software
- marketing and/or sales skills.
This is a casual position and hours will be on an as required basis. Salary will be in the range of $32.11 to $35.13 per hour plus 25% casual loading based on skills and experience plus superannuation. Attractive salary packaging provisions are available to increase take home pay.
