The organisation are a NFP provider of specialist social and disability services across Victoria. About the role

The aim of this role is to work with the large already well established team of support coordinators (25 +) to provide a high standard of initial contact services and support coordination services. All of the support coordinators go above and beyond to provide assistance to strengthen our customers’ abilities to coordinate and implement supports and participate fully in the community. This support could involve providing initial support to help participants link up with service providers in their local areas, help access new services both NDIS funded and mainstream services, coordinate the provision of supports, resolving issues complaints and assisting in times of need or crisis as well as assisting participants to work towards independence and the ability to participate fully in their communities.

Support coordination may form a small part of an NDIS plan for those with the ability to eventually become more independent however some participants may rely on complex support coordination on a long term, ongoing basis. The exact nature of each participants supports and needs will of course vary dramatically and it’s important to remember that client centred approach and individualise service be provided.

  • An understanding of the NDIS and challenges faced by those with a disability
  • Working knowledge of disability services and therapeutic organisations locally
  • Great organisational skills and computer literacy
  • A Qualification in health, allied health or social care would be highly regarded
