PART TIME RN/EN CARE COORDINATOR - GRIFFITH
Home at Heart is an Approved home care provider, located in Wagga Wagga and Griffith. It provides home based support so that people can remain living independently. We are passionate in supporting people to choose how they want to age in their own homes.
Home at Heart has an opening for an experienced care coordinator in Griffith. This role is integral to the successful delivery of direct care services to clients of community home based care services.
In this position you are responsible for overseeing care of clients receiving both high and low-level Home-Based Programs and facilitate the care with services based on their needs. You will achieve this through coordinating the team and clients and providing quality day to day assessments, support plans, clinical care, supervision and mentoring.
This position will suit someone who enjoys working with families and wants to support the elderly in staying in their own homes longer. As an effective, proactive member of the management team you will also work to ensure that Home at Heart meets its goal of providing efficient professional service and support of the highest standard for all its clients.
Experience in coordination and community care is required for this position. This position is part-time with the possibility to become full-time.
THE SUCCESSFUL CANDIDATE WILL:
- Coordinate all home-based client care
- Use your clinical skills to assist and manage client care
- Enjoy liaising & supporting clients and their families
- Coordinate/Case Manage, the care delivery and goals for our clients
- Strong attention to detail and problem-solving skills
- Be a part of a professional multi-disciplinary team
TO BE A SUCCESSFUL CANDIDATE:
- You must have industry experience
- Be competent using technology
- Possess exceptional communication and organisational skills
- Be able to time manage and prioritise workloads
- Have community-based rostering and coordination experience
- Can work autonomously and in a team environment
- A current Registered Nurse or Enrolled Nurse is advantageous
- Have a current National Police Check, NSW Drivers Licence and own car.
