NDIS Transition Officer
Fixed-term full-time position (6 months)

With over 25 years’ experience in providing services to people with disability it is our aim to support and empower people to achieve their goals by delivering inclusive and personalised services.
We are currently seeking to assist our clients’ smooth transition to the NDIS by engaging an NDIS Transition Officer. This role will be responsible for supporting existing Mosaic clients and their families to prepare for the NDIS so that they are able to successfully navigate the transition to this new model.
The successful candidate will meet both in house groups and 1:1 with existing clients and families. Responsibilities will include educating families in the new system and its potential impact, working with families and staff to prepare documents to ensure no supports are overlooked, realigning existing individual plans to the NDIS requirements, and ensuring supports can be claimed as ‘reasonable and necessary’. There may also be scope for:
- Supporting clients in the planning meeting if invited by families / client to do so
- Linking clients with independent advocates where they have complex needs/no family or independent support
- Supporting clients to appeal plans should a possible disadvantage arise after their new NDIA plan is developed
The successful applicant will have;
- Experience in working 1:1 and forming close professional relationships with people with disability and their families.
- High level expertise in all Western Australian disability funding systems
- Experience in working with the NDIS price guide
- Strong people skills
- Strong linkages to the WA disability sector
