About the Role:

An exciting opportunity has arisen at our Bunbury office, working with our friendly team as a Partners in Recovery (PIR) Support Facilitator on a Part-Time basis (38 hours per fortnight). The successful applicant will support our customers.by;

  • Demonstrating a commitment to principles and practices of recovery in all dealings with mental health consumers, carers and stakeholders of the PIR program;
  • Undertaking initial assessments of participant’s service and support needs using the nominated needs assessment tools, and/or engage specialists where required;
  • Making referrals to agencies and services as appropriate;
  • Undertaking coordination activities for eligible participants across multiple agencies;
  • Developing, monitoring and regularly reviewing the participant's PIR Action Plan in consultation with PIR participant, carer and other stakeholders to identify changes in support needs;
  • Facilitating care-coordination meetings;
  • Showing a genuine commitment to helping people with mental illness related functional limitations, a capacity to relate to them with dignity and respect, and adopting person centred practices.

The successful candidate would be able to demonstrate the following attributes:

  • Knowledge of mental illness and skills in working with people experiencing mental illness;
  • Effective listening and communication skills with an ability to be creative with problem solving;
  • Knowledgeable about when to seek help or supervision and how to work in a team environment;
  • Treats customers, carers and staff with dignity and respect, and balance the right to privacy and confidentiality with duty of care;
  • Ability to analyse and identify participant needs and address these through innovative and collaborative methods;
  • Delivery of exceptional levels of service to customers, team members, and other stakeholders;
  • Ability to work both independently and in a team;
  • Highly organised and excellent time management skills;
  • Ability to identify persons at risk and facilitate a safety plan;
  • Excellent record keeping skills in relation to interaction with customers and agencies.

Essential Criteria:

  • Current National Police Clearance with 6 months or more currency or willingness to apply for and provide one prior to commencing employment; ;
  • Current Australian Drivers Licence with no restrictions;
  • Current Working with Children Check Card;
  • Current First Aid Certificate with 6 months or more currency or willing to obtain.
  • Minimum Certificate IV in Mental Health or relevant qualifications;
  • Relevant experience in mental health community support;
