BBCR is a leading community care provider offering a range of services across all ages in the Shoalhaven and Illawarra. We are seeking a highly motivated and experienced Case Manager to join our team to manage a case load of BBCR clients receiving services through our Aged Care and Disability programs. BBCR has a range of clients funded under different programmes such as, Commonwealth Home Support Program, Home Care Package, Short Term Restorative Care, Community Visitors Scheme and the National Disability Insurance Scheme.

 

The Case Manager will report to the Team Leader responsible for BBCR clients in the Southern Shoalhaven. You will be accountable for accomplishing quality client care and satisfaction. You will achieve this through working with each client to provide coordination, case management, and gain client goals as outlined in their Support Plans. This role is available on a flexible full time basis, and will be based in the Southern Shoalhaven area (Sanctuary Point).

Essential criteria:

The successful candidate will have:

  • Experience providing case management and care coordination services in the community care sector
  • Experience working with people with disability, older people who are frail, and their carers
  • Relevant tertiary qualification
  • Experience managing client NDIA/HCP packaged budgets
  • Demonstrated ability to work autonomously, reporting, and taking direction from a Supervisor as required
  • Experience in achieving annual KPI’s and complying with all legislative requirements
  • High level of communication, verbal, written and IT skills
