Part-time: 22.5 hours per week

Creating Links is seeking a highly experienced, vibrant and committed person to join the team as the Aged Care Coordinator working from our Bankstown Office. This role is being offered as a six month contract with an option to renew based on growth targets being achieved.

The Aged Care Coordinator will be responsible for the expansion and coordination of our Aged Care services (including TACP), providing expert advice and care to our clients. You will lead and develop a team of Community Care Workers in conjunction with the Community Care Services Manager and be committed to continuous quality improvements, striving to promote and provide best practice in all areas of your role.

Essential Criteria

Qualifications

  • Tertiary qualifications in Community Welfare, Aged Care/Health or related fields
  • Current drivers licence with no restrictions

Skills and Knowledge

  • Demonstrated experience in a team leadership role including workload management, performance management and critical and reflective thinking skills in decision making and problem solving
  • Minimum of five years' practical experience within provision of quality Aged Care services in a management role
  • A comprehensive clinical knowledge, particularly to Age related health issues
  • Ability to communicate with a range of people including older people, staff, volunteers
  • Demonstrated understanding of quality assurance and continuous improvement
  • Demonstrated understanding of the Aged Care sector and contemporary issues relating to the Aged Care Industry
  • Working knowledge of Community Aged Care funding and home care packages
  • Good written communication skills including experience in preparing routine documentation related to care notes, analytical and critical thinking skills
  • Demonstrated commitment to continual professional and personal development
  • Post basic qualifications relevant to Aged Care
  • National Criminal Records Check
  • Valid Working with Children Clearance
  • Current First Aid Certificate

Desirable Criteria

  • Fluency in a second relevant community language as well as English
  • Previous experience or knowledge of working in or with 'practice groups' or 'communities of practice'
  • Relevant Tertiary qualifications in Management or working towards same
  • Membership in Industry Reference Groups, Committees and Associations
