About the organisations

McArthur's RAS Assessment locums are deployed on assignments in all areas of Melbourne, they can range from short term (at least one month) to Long Term (6 months plus) with some of these roles leading to permanent employment offers. Our Assessors are working alongside dynamic, multi-disciplinary teams, working with the elderly, frail and disabled, providing clients with services to allow them to remain living independently at home and feeling supported and socially connected. The Home Support Assessor is an exciting role undertaking the assessment of older people in their homes and supporting their access to needed services.

Duties

Home Support Assessors provide basic home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the National Screening and Assessment Form (NSAF). Working with each client, the Home Support Assessor establishes goals specific to the client; develops appropriate strategies and actions, including referral to age care and non-aged care services; and develops a Support Plan. The Home Support Assessor completes the NSAF and matches and refers clients to appropriate service(s).

Skills and experience

If you have had prior experience in Community Care, performing home based Assessments within the Aged Care sector, either in Local government or not for profit organisations, we would love to speak with you about how we can help you to achieve your career or lifestyle goals. You will need a sound understanding of MAC use, HACC PYP and CHSP funding, and hold a tertiary qualification in Allied Health, Nursing or Social Sciences. You will need to be able to to commit to 3-5 days per week. A police check dated within the last twelve months is essential. Completed SAO2 training is preferred but not essential.
