Diversicare is excited to be expanding on our existing Regional Assessment Team requiring additional Assessors to ensure we can complete Commonwealth Home Support Program Regional Assessments for the CALD Community across Southeast Queensland.
We are seeking casual Home Support Assessors based in Toowomba to perform client assessments using the national screening system. You will make client service recommendations and design support plans for clients to achieve the clients' health goals. Some short term case management will be required.
These position will work closely with other members of Diversicare's multicultural assessment and service delivery team and in close consultation with other service providers.
The position will allow flexible working arrangements including working from home and there may be some flexibility in working hours.
Selection Criteria:
Essential
- Must hold as a minimum a Certificate IV qualification in Aged, Home & Community or Disability Care.
- Minimum of 2 years experience working within the community care industry.
- Demonstrated ability to work independently and multitask with a can do attitude in a multicultural team environment and highly mobile position.
- Demonstrated excellent communication skills with strong proficiency and comprehension in English.
- Demonstrated high level computer skills including use of Microsoft Office Suite programs.
Desirable
- Completion of the Regional Assessment Service units of competencies as listed below:
- CHCAGEOO1 - Facilitate the Empowerment of Older People
- CHCCCSOO5 - Conduct Individual Assessments
- CHCCCS0I6 - Respond to Client Needs
- Demonstrated skills in assessments and case management for low and complex clients with a wellness and re-ablement focus in the community, health and aged care sector.
- Demonstrated knowledge of Services and referral pathways within Southeast Queensland.
- The ability to speak a second language will be a definite advantage.
- Demonstrated understanding and commitment to the provision of care for clients from CALD communities.
The successful person will also require a satisfactory National Police Check, a current Queensland Driver's licence and access to a road worthy and insured vehicle for work purposes.
