Your new role

As a case manager you will deliver effective care and support for clients that enhance self-determination, capacity and resilience, as well as improving functional independence, social engagement and quality of life for the client and family carer. You will provide cost effective, evidence based and goal orientated case management support for clients delivering positive outcomes to a clients quality of life.

What you'll need to succeed

  • Degree qualifications in Allied Health, Nursing or other relevant field
  • Certification with the Case Management Society of Australia
  • Previous experience in an allied, aged-care or community aged care environment
  • Current Drivers Licence
  • Current First Aid certificate
