This is a new opportunity based in our Ashfield Office and you will report to the PIR Team Lead in our Partners in Recovery team.

 

This is a maximum term contract ending 30 June 2019 with the potential for extension.

 

Day to Day duties

 

  • Facilitate the timely, supportive, inclusive and recovery orientated coordination of services for people with severe and enduring mental illness.
  • Work with consumers, their families and decision-makers to increase participation and inclusion based on the individual’s goals and choices
  • Develop strong working relationships with key external stakeholders including other service providers
  • Support a caseload of consumers to apply for the NDIS, increase their capacity to understand, monitor and manage supports of their choice.
  • Support a caseload of consumers to implement their NDIS plan, negotiate and establish agreements with New Horizons and other providers.
  • Ensure continuous quality improvements are maintained and implemented

 

 

What you need to have:

 

 

 

  • Passion, Respect & Integrity
  • Relevant Tertiary qualifications and/or significant knowledge and experience in a similar role
  • Understand the NDIS and ability to adapt to changes quickly
  • Self-motivated with the ability to work autonomously and also in a team environment
  • Ability and confidence to communicate and work with other staff outside of the immediate team
  • High level of accuracy and attention to detail
  • Ability to multi-task and manage competing priorities according to agreed timeframes and quality standards
  • High level written and verbal communication skills
  • Demonstrated skills in working with and knowledge of Microsoft Office programs including Word, Excel, PowerPoint and Outlook
  • Strong stakeholder management and consumer service skills
  • Demonstrated planning, time management, organisational and problem solving skills
