Vision Australia is a leading national provider of blindness and low vision services in Australia. We work in partnership with Australians who are blind or have low vision to help them achieve the possibilities they choose in life.

We support more than 27,500 people of all ages and life stages, and circumstances. We do this through 28 Vision Australia centres in Victoria, New South Wales, the Australian Capital Territory, Queensland, South Australia and Western Australia; 30 clinics; and through outreach programs in the Northern Territory and Tasmania.

We are seeking an experienced Case Manager to join our Home Care Packages team in the South East Melbourne region. The successful applicant will utilise their specialist knowledge and skills to work as part of a team to coordinate the delivery of our Home Care Packages.

You will provide support for our consumers to assist them to achieve their goals and aspirations and enable them remain at home for as long as they choose to do so. This position also involves engaging with internal and external stakeholders including service users, brokered service providers, My Aged Care, health services and community based organisations.

Recent experience in providing case management in the Home Care Packages area is essential.
