Purpose of the Position
HYAP provides case management, mediation and practical support working with young people (aged 12- under 16) who are homeless or potentially homeless, and their families.
Key objectives of HYAP include supporting children and young people under 16 years old to:
- Rebuild family, kin and cultural connections, and work towards family reconciliation where appropriate
- Successfully transition to independence
- Engage with education, training and/or employment
- Access mainstream health, mental health and wellbeing services
- Engage with the broader community to support their successful transition into independence
This position requires well developed skills and knowledge of relevant interventions which may assist vulnerable Aboriginal and non-Aboriginal families and young people and people from diverse cultures and communities.
Service provision includes outreach to smaller town centres within the catchment and some on call requirements.
