About the Organisation

Based in Parkville, A program jointly funded by the Commonwealth Department of Health & Ageing and the State Department of Human Services, TCP provides home-based packages and residential-care-based packages.
The Program provides short-term support and active management for older people & the elderly at the interface of the acute & sub-acute and residential aged care sectors. The Program is goal oriented, time limited and targets older people who have been recently discharged from hospital and require more time & support in a non-hospital setting to compete their recovery, optimise their functional capacity and finalise & gain access to longer-term care arrangements.

Duties

  • Case Management, referral and advocacy for bed based and home based TCP clients
  • Care planning, documentation
  • Liaise with clients, families, stakeholders to ensure best outcomes for your clients

Skills and Experience

Previous TCP Case Management experience in either / both Home and Bed Based TCP is essential to this role. As this is a locum role, applicants must be experienced with working across Transitional Care and possess a tertiary qualification in Nursing, Allied Health or Social Sciences. A social work background will be highly regarded for this particular position.
