About the Role

Due to our extension of funding we are looking for 3x Outreach Case Workers, who would like to work within AOD and Mental Health space.

Your Duties

  • Respond to referrals, obtain informed consent, conduct bio-psychosocial assessment, surveys and registration paperwork.

 

  • Establish therapeutic alliance with participants and deliver client focussed AOD/MH and generalist counselling/therapeutic interventions to individuals and groups

 

  • Develop recovery focussed, client directed, AOD/MH support plans inclusive of exit planning, aftercare and referral to supplementary services.

 

  • Provide case management and coordinated care for each client, supporting facilitated referral and access to acute treatment services for those with a high level of AOD dependence, and those at risk of suicide

 

  • Deliver engaging AOD harm reduction education/service promotion at school assemblies, community forums and events.

 

  • Collaborate with school support staff, family/carers, and existing community based partners/programs in accordance with client needs

     

Experience

 

  • Relevant qualifications and experience in youth & community services, mental health, alcohol & other drugs

 

  • Working knowledge of alcohol & other drugs, and mental health issues in youth populations

 

  • Strong written and verbal communication skills.

 

  • Sound interpersonal skills

 

  • Ability to work from a culturally informed and strengths based framework.

 

  • Strong conflict resolution skills.

 

  • Ability to work independently and as part of a team, using personal initiative and problem solving techniques to achieve workplace solutions.

 

  • Able to travel and provide outreach case management in school and community settings.

 

  •     Current Driver’s Licence

 

  • Ability to coordinate, facilitate and participate in group and one-on-one counselling sessions and activities

 

  • Understanding of duty of care

 

  • Excellent computer software skills, specifically MS Office

 

  • First Aid Certificate

As part of our Employment screening process you will be required to undertake a National Criminal History Check.
