We are currently seeking an experienced Support Planning Coordinator who will focus on the development of individualised client service plans within our Specialist Services team based within our Mackay region.

 

Your new role

This highly independent part-time Support Coordinator will develop individualised client service plans Support Plans to support persons with a disability.

You will be required to provide advice, guidance and support to Multicap Services in the Mackay region. In the creation of our support plans, you will ensure that the customer's aspirations and goals are clearly captured and tangibly linked to the service and supports delivered.

Your responsibilities will include but are not limited to:

  • Developing in partnership with all key stakeholders service plans for individuals accessing Multicap activities in accordance with their assessed needs.
  • Providing support to individuals, family members and other stakeholders to ensure that planning processes are fully consultative and responsive to views and aspirations of key decision makers.
  • Providing advice, guidance and support to staff teams to ensure that individuals, particularly those with complex needs have plans that are effective, current and reviewed appropriately
  • Supporting monitoring of current service plans for Multicap customers, their families and staff as required
  • Supporting the work of the organization through training and/or other means to develop and maintain on-going best practice in individual planning processes
  • Developing and maintaining mutually beneficial working relationships with staff, internal groups and external bodies to establish viable interactive partnerships

 

This role will require a high level of autonomy, attention to detail and the ability to encourage interaction and discussion with a variety of internal stakeholders.

This is a permanent, part-time position of 38 hours per fortnight.

 

What you bring

To be considered for the position, you will ideally have a positive attitude, enjoy working as part of a high functioning team and be able to work flexibly on a range of projects, including:

  • Qualifications in human services or social sciences or equivalent (Desirable)
  • Possession and demonstration of sound disability (or allied health/welfare/education field) experience, knowledge and/or training relevant to the position
  • Demonstrated experience coordinating the provision of support to people with a disability and their families
  • Knowledge of the State Disability Services Act and the 2008 Amendment relating to restrictive practices
  • Demonstrated high level of interpersonal  and communication skills to develop relationships with team members and stakeholders and deal with issues of a sensitive nature
  • Demonstrated problem solving skills to successfully identify problems, develop solutions and implement these using a logical and systematic approach
  • Understanding of positive behaviour support frameworks
