Nextt’s Home Care Package Program is delivered on a Consumer Directed Care (CDC) basis, underpinned by a strong emphasis on wellness and re-ablement and provides ‘opportunities’ for consumers to have greater control over their own lives by acknowledging clients'right to make choices about the types of care and support services they can access and the way in which these services are delivered. 

 

About the Role:

Due to continued growth in our aged services programs we require additional Case Managers within. There are multiple opportunities full time, part time (min 3 days per week) and casual roles available.

You would be part of an enthusiastic and inclusive team where creativity, innovation and advanced case management skills are highly valued.  Reporting to the Program Manager, the successful applicants will work closely with the other Case Managers and a specialised team based out of our Collins Street office in Melbourne. 

The role of the Case Manager is to assist eligible older people to live independently in their own homes and in the community focusing on their wellness through restorative and re-ablement practices by delivering flexible, innovative and client focused support. The role would suit someone who has a client centric approach towards supporting elderly people who wish to retain their independence and quality of life. The role requires someone with comprehensive experience within Aged Care Case Management specifically in HCP, CHSP, DVA, MAC and Medicare portals and financial packaging. 

The successful candidates would be up for a challenge in a dynamic market and flexible to work through organisational changes. 

Key Responsibilities:

  • Develop support plans and ensure appropriate services are provided to meet clients' needs in line with both Nextt and the Department of Social Services guidelines, including CDC.
  • Determine, plan, monitor and manage the most appropriate service requirements in partnership with the client and family/support network.
  • Liaise with other service providers and allied health professionals to promote a coordinated and seamless approach to service provision for clients.
  • Ensure adequate and appropriate documentation is maintained for each client and comply with reporting and administrative requirements, and ensure that all data is accurate and up to date.
  • Undertake regular reviews, monitor all changes to client requirements, and respond according to agreed expectations.
  • Ensure compliance with national and international standards and legislation and promote performance improvement throughout the organization.
  • Develop positive working relationships with ACATs, key referrers and government bodies and attend meetings and information sessions as required.

Skills and Experience:

  • Relevant qualifications and knowledge (tertiary qualifications in nursing, allied health, social work or similar)
  • Experience in Aged Care - HCP would be desirable
  • Knowledge of the community aged care sector, particularly Home Care Packages is preferable
  • Skills in client assessments, care and goal planning and risk management
  • Good budgeting skills
  • Excellent communication and negotiation skills
