About the Opportunity
St Carthage's Community Care has an exciting full-time opportunity for a Support Planner to join their dedicated Aged Care Team in Lismore, NSW.
Reporting to the Community Services Manager, you will be responsible for the delivery of integrated, person-centred services that are flexible, innovative, and consistent with the objectives identified in the strategic plan.
You will also contribute to the organisation's strategic plan through the delivery of quality and innovative client services and liaise with a variety of internal and external service providers/agencies in relation to supporting people in the community.About You
