SUPPORT CO-ORDINATOR / CASE WORKER
Full Time
Two (2) Year Contract
We are seeking a motivated and resourceful person to work creatively supporting people with an Acquired Brain Injury to become more independent in life. The team at Headstart ABI Service assists people re-develop life skills and renew community involvements. You will be responsible for initiating and co-ordinating new opportunities and providing a range of diverse and individually tailored supports under the NDIS for individuals living with an ABI. You will be working with a team committed to the concept of individualised supports, community inclusion and consumer driven services.
Essential:
-
Tertiary qualifications in health, welfare, or similar
-
Minimum 2 years experience in case management
-
An understanding of the effects of brain injury on the psychological, social, physical and cognitive functions
-
Understanding of the purpose and functions of the National Disability Insurance Scheme (NDIS)
-
experience in the design and implementation of supports and services around individual needs
-
ability to work effectively with families and extended social supports
-
working knowledge of the individualised planning processes
-
excellent written communication,and the ability to prepare detailed reports and assessments
-
well developed interpersonal skills including leadership, negotiation and conflict resolution
-
demonstrated experience in using these skills in dealing with complex issues and sensitive situations
-
the ability to communicate with a wide range of individuals, families and services
-
skills in support and supervision of direct staff
-
ability to prioritise and co-ordinate a complex case load
-
working knowledge of the National Disability Service Standards

skills in MS Office, rostering and Client Management Systems (CIMSAbility)

First Aid certificate / Working with Children Check/ National Police Check

Driver's licence
Salary: SCHCADS Award Level 4
This position may become permanent dependent upon funding. This position is based in Hamilton NSW.
