IFYS currently has a Team Facilitator position available in our Inclusive Living Program. The Team Facilitator acts as a conduit to provide information to the Client Services Coordinator and works with Disability Support Workers in the daily operations of the program. If you are a passionate, person centred, flexible and professional Practitioner in the field of Disability Services, then this is the job for you.

The key responsibilities for this role include:

  • Provide, monitor and support daily operational functions to ensure a safe and secure environment.
  • Support Consumers to achieve agreed goals in line with the Positive Behavior Support Plan (PBSP) and/or Personal Plan with a high level of casework support.
  • Monitoring and implementation of Positive Behaviour Support Plans
  • Supervision of employees
  • Staff training and development
  • Assist the Client Services Co-ordinator with rosters, reporting, management of budgets and overseeing staff.
  • Research to develop and maintain the activity planning and facilitate a high level of community involvement for the Consumers.
  • Maintain collaborative partnerships and linkages aimed at improving client outcomes.
  • The timely and accurate completion of administrative duties.
  • Support consumers and staff transition to NDIS Supports by Design
  • Procedural and process implementation

The key skills required for this role include:

  • Previous experience as a disability support worker
  • Have previous experience managing complex behaviours with positive behaviour support;
  • Computerised documentation and maintenance of service and client records;
  • Flexibility and ability to work as part of a State wide team.
  • Effective communication skills
  • Effectively Lead and manage teams
