What do we do?

The Institute for Urban Indigenous Health (IUIH) was established to provide a coordinated and integrated approach to the planning, development and delivery of primary health care services to Aboriginal and Torres Strait Islander populations within the South East Qld Region. The IUIH is a lead agency working in partnership with key stakeholders to support the effective implementation of the Council of Australian Governments ‘Closing the Gap’ initiatives.

This role: The primary purpose of this position is to provide direct care to the eligible frail aged members of the community in their own home. The position provides assistance with tasks such as showering, toileting, dressing and shaving. This role may also include domestic assistance, social support, meal preparation and shopping. The role will be required to perform in accordance with Client Care plans, the Community Care Common Standards and the IUIH Policies and Procedures.

The position will be responsible for:

  • Assist with general client care, including undertake personal care, domestic assistance, social support, respite, meals and transport activities
  • Provide daily assistance to clients by supporting them to maintain their independence to remain in their own home
  • Provide personal care assistance to clients in their homes, according to the assessed needs as specified in their care plans (e.g. bathing, showering, grooming, nutrition, oral hygiene), in line with level of Certification and standard operating procedures
  • Provide domestic assistance and social support services to clients if required
  • Provide quality care at all times
  • Relieve other domestic assistance / Home Care/Respite Staff as required
  • Apply first aid when required
  • Carry out extra duties as required
  • Assist in the planning, implementing, reviewing and evaluating programs
  • Assist in reviewing client care plans and update client information when required
  • Check equipment regularly and report any faulty equipment to the Team Leader

To be successful in this role, you will have:

  • Certificate III or IV in Aged Care or other relevant qualification (or willingness to completed)
  • Current First Aid or willingness to obtain
  • National Police Clearance
  • Ability to travel across designated service region as required
  • Current C Class Driver’s Licence (QLD)
  • Current First Aid or willingness to obtain
  • Own ‘private’ reliable vehicle with proof of registration
  • Physical fitness to carry out duties allocated
  • Minimum six to twelve months’ experience in a similar home and community care role
