 About the role...

Aftercare has an exciting opportunity for a Support Facilitator to join our Inner West Sydney Partners In Recovery (IWSPIR) team.

Using a person-centred, trauma-informed, and recovery-oriented approach, IWSPIR Support Facilitators actively engage with individuals to facilitate networks of services and supports that are more collaborative, coordinated and integrated. They collaborate with the individual and those around them to design sustainable strategies to manage complex day-to-day challenges and enhance individualised recovery processes. Support Facilitators work to improve the systemic responses and outcomes for people experiencing severe and persistent mental illness who have complex needs, by building the capacity of the people they work with and the services that support them.

As part of Aftercare's IWSPIR team you will:

  • Facilitate better coordination of clinical and other supports and services to deliver 'wrap around' care individually tailored to the person's needs.
  • Strengthen collaborative partnerships and build improved links between various clinical and community support organisations.
  • Assist individuals to access and navigate transition to the National Disability Insurance Scheme (NDIS)
  • Promote a community-based, person-centred, trauma-informed, and recovery-oriented model to underpin all clinical and community support services delivered to people experiencing severe and persistent mental illness with complex needs.


The Right Fit...

 IWSPIR Support Facilitators come from a diverse range of backgrounds and disciplines. The successful candidate will ideally have:

  • Tertiary qualifications in a related mental health, social sciences, community services or welfare discipline.
  • Experience working with people with severe and persistent mental health issues in a community and/or outreach setting.
  • Demonstrated capacity to relate to people experiencing mental illness and all other stakeholders with dignity and respect.
  • Understanding of the principles of duty of care, rights to privacy and confidentiality.
  • Basic MS Office skills.
  • Current Driver's Licence.
