The Stand by Me Program is an intensive case support service for young people aged 16 and over that reside in the out of home care system. Through intensive personal case support and assistance with accommodation, the Stand by Me Program will ensure a more successful transition to adult living for young people leaving care. The Program has a focus on care leavers who are most at risk of homelessness, taking an early intervention approach.

The successful applicant will have experience working with young people in care and be passionate about building strong relationships in order to be able to develop their independent life skills. You will have strong advocacy skills with the ability to act as a conduit between young people and relevant support services. You will be a strong communicator with good problem solving skills.
