Intensive Case Management Initiative (ICMI) – Part Time position

Case Worker

  • Part-Time 0.8 (4 Days Per Week, days to be negotiated)

We seek an energetic professional with a passion for working with homeless adults, females, males and couples (18-35 years) who is committed to providing innovative, flexible and accountable service.  New graduates welcome.

Main Duties:

The ICMI worker role is a single person program providing either case management secondary consultation or co case management support to participants supported by COS programs. This program is located at Community Outreach Services (COS) Kensington site.

The ICMI case worker will work collaboratively with other direct service staff in the Adult Service Network and other social service and mental/health sectors to ensure clients are provided with appropriate support in facilitating the recovery of people with an acute mental illness.

The ICMI case worker is funded to provide case management to a case load of 5-6 participants depending on complexity and support needs. Caseloads for part-time positions are adjusted accordingly.
