Centacare is the social services arm of the Catholic Archdiocese of Brisbane and has been operating in South East Queensland for over 50 years. 

Centacare currently provides services to over 100,000 clients per year, and currently employs over 3000 workers. A multi-faceted organisation, Centacare provides a broad base of social services including: family relationship services, disability services, community services, aged care, pastoral ministries, and child care services including outside school hours care.

Centacare Community Services directorate (CCS) is structured into 6 regions with a centralised support service delivering aged care, mental health, housing support, homelessness, respite and transition, independent living and support for people with a disability and learning and leisure services. 

The job:

Centacare Community Services has 2 full time fixed term Support Coordinator positions available, with a contract end date of 31 July 2019. The primary purpose of the role is to work creatively with clients and/or carers to explore opportunities and build their capacity to utilise NDIS plans to pursue their individual goals.

In this role you will:

  • Identify community, mainstream, funded and informal supports to assist the individual to pursue their goals as outlined in their NDIS plan;
  • Assisting in choosing preferred options and providers; negotiate services and service agreements;
  • Maintain networks with relevant professionals, government agencies, NDIA and other pertinent organisations; 
  • Arrange assessments to determine participant need; 
  • Develop budgets to manage plans; 
  • Provide participants with assistance to resolve problems, understand service agreements and change or end service agreements; 
  • Maintain accurate case notes and billing information;
  • Report on services, including collation of statistical information; 
  • Maintain accurate case notes; and
  • Contribute to continuous quality improvement in the provision of services.

What we need from you:

  • Relevant tertiary qualification (e.g. Social Work or Behavioural Science) and/or several years of relevant industry experience; or
  • Post Graduate qualifications; or
  • Associate Diploma/Less formal qualification with substantial years of relevant industry experience;
  • Knowledge and understanding of client plan funding;
  • Advanced understanding of all aspects of NDIS will be highly regarded;
  • Intermediate level skills in the use of Microsoft  Office suite of products;
  • Intermediate level skills in the use of TRACCS or other client information management system;
  • Knowledge and understanding of Disability Services Act 2006, Aged Care Act 1997 and Mental Health Act 2016;
  • Knowledge and understanding of National Aged Care and Disability Services Reforms and implications on community services;
  • Strong communication skills, both written and verbal;
  • Ability to work within minimal supervision, exercising initiative and professional judgement;
  • Ability to prioritise workloads to meet schedules through high level time management skills;
  • Strong leadership qualities with a high level of tact, integrity and discretion;
  • A valid Queensland drivers licence;
  • A National Police Certificate; and
  • A current Blue card and Yellow card exemption, or willingness to obtain.

The benefits

You will be paid in accordance with our Collective Agreement with the base salary up to $75,127.52pa ($38.02 p/h) plus superannuation depending on qualifications and/or previous experience.
