About the Role

The Intensive Support Coordinator manages a portfolio of severely injured clients, with highly complex individual situations, to achieve maximum independence and participation in all life areas. Your clients will be at varying stages of rehabilitation and will often have additional factors affecting their recovery, so you will be required to coordinate multiple stakeholders and medical, rehabilitation and disability organisations.

You will have the opportunity to work in a supportive, social and collaborative team, and as this position is still relatively in its infancy, contribute to the development of the role.  This role will be three days per week and can either be based in Melbourne CBD or South Eastern Suburbs.

Please note there will be regular travel to Geelong required, including the initial training period.  You will also need a current Victorian drivers licence.

About You

To be successful in this role, you will ideally possess the following skills and attributes:

  • Experience in a relevant industry, including care coordination, outreach services, social work/case management, mental health, disability or rehabilitation
  • Experience in long term individualised planning, goal setting and service provision, to achieve optimal client outcomes
  • Strong leadership skills, resilience and a proven ability to exercise good judgement
  • Excellent stakeholder engagement and communication skills

Our Values

  • We value life
  • We make every conversation count
  • We will find a better way, today
  • We make the complicated simple

Diversity and inclusion

The TAC promotes a workplace that actively seeks to include, welcome and value unique contributions of all people and is committed to diversity and social inclusion in its employment practices. As Victoria’s Commission for transport accident treatment and services, we strongly encourage people with a disability to apply for our roles.  People from indigenous, culturally and linguistically diverse backgrounds are also encouraged to apply.

How to apply

Click APPLY or contact Sarah Middleton on 03 5225 7787 for further information.  Applications for this role will be accepted up until 5.00pm Friday 27th July 2018.
