About the Role

We are currently seeking an experienced Care Planning Coordinator to join our team. This role is a full time contract role to 30 June 2019, with the possibility of extension dependent on funding.  Be responsible for providing high quality support to carers contacting Carers ACT. Duties of these roles include:

  • Providing information and resources to carers on Carers ACT services, as well as basic information on external providers;
  • Undertaking needs assessments of individual carer needs and developing care plans;
  • Implementation of appropriate services and supports for carers including liaising and negotiating with service providers;
  • Organising outreach activities and workshops;
  • Working with carers to empower and support them to navigate the care system;
  • Maintenance of accurate client and service provider records.

 Skills and Experience

  • Relevant qualifications in health, mental health, aged care or disability fields;
  • Experience and/or qualifications in the delivery of care planning, case management or intake and assessment processes;
  • Demonstrated understanding of carer issues in the community;
  • Highly developed communication and interpersonal skills with a demonstrated empathetic and flexible approach;
  • Well-developed record keeping experience;
  • Excellent computer skills including proficiency in Microsoft Office suite and use of databases;
  • Current driver's license;
  • Current Working with Vulnerable People registration and National Police Check, or the ability to obtain.
