Your new role

As part of the Healthy Communities/ NDIS Team at Merri Health, your main responsibilities will include:

  •  Assist NDIS participants to implement the supports and services within their NDIS plans
  • Model and embed a participant directed approach which promotes maximum choice and control for NDIS participants
  • Strengthen and enhance NDIS participants to coordinate their own supports which enhances community inclusion
  • Advocate for participants to access services and supports, especially regarding their education, capacity and resilience building
  • Meet program service delivery targets as reasonably expected within the role

This is a full time role offered on a fixed term (30/06/2019) basis. You will be based in Coburg. In order to be considered for this position, you must have the right to work in Australia. 

What you’ll need to succeed

  • A relevant Qualification ie: Social Work, Certificate IV in Disability or Mental Health
  • At least two years’ experience working with vulnerable people particularly people with psychosocial disabilities
  • Experience in planning, case management, coordination and review for people with complex needs
  • Advanced communication and interpersonal skills
