This role plays a critical part in assisting our senior clients to lead independent, healthy lives by undertaking holistic evidence-based assessments, goal-driven support planning and case management. Therefore, we're looking for someone who is experienced in completing assessments and care plans but also highly motivated to making a real difference in people's lives.

The successful candidate will be an excellent communicator who genuinely listens and takes the time to understand our client's needs. Exceptional interpersonal skills are imperative and this also must go hand in hand with strong IT skills.

In order to be considered for this role you must live within the Illawarra region as this is a community based role which will see you travelling to our client's home located within this area. Visit the DSS Aged Care Planning Region website to view the map for more information.

Essential criteria for the role are as follows but not limited to:

Professional experience:

  • Home Care Packages (HCP) or Regional Assessment Service (RAS)
  • Case management, managing individual budgets and attending assessments with clients that include support planning and goal setting in aged, community, health or social services.
  • Working remotely with a high degree of drive
  • Preparing reports, contributing to operational plans, conducting and facilitating presentations

Personal attributes:

  • A confident and skilled communicator, able to engage and motivate clients, families, staff and other stakeholders with a positive approach.
  • Ability in networking and developing working relationships with referral agencies, health services and community groups

Qualifications and requirements:

  • Registered Nurse or Allied Health Professional such as a Physiotherapist/OT OR
  • Hold a relevant certificate in Aged Care/Community/Case Management.
  • The ability to attend 15 days of initial training and orientation in our head office at Coolangatta, QLD, and 10 days of training from home (5.5 weeks of full time training all paid for)
  • Hold an unrestricted Australian driver's licence.
  • Have a stable internet access (as you will be a virtual worker).
  • Ability to travel and stay away from home at least 2 days every month.

This is a fixed term 12 month contract, further employment opportunities and advancements may also be available at the end of this period.
