Department of Communities, Disability Services and Seniors/Strathpine & Caboolture

  • Temporary - Flexible work hours full time until 30/12/2018
  • Salary in the range of $58,573 to $86,000 per annum excluding superannuation

 

Accountabilities:

  • Undertake comprehensive specialist disability assessments and develop individualised plans for clients living with a disability who have moderate to complex support needs and their families and carers within an inter-disciplinary environment, including clinical case management.
  • Using collaborative practice and a person centred approach work with people with a disability, their families, carers and other agencies to implement intervention plans within agreed timeframes.
  • Monitor, evaluate and review individual intervention plans and programs against the person’s needs and goals according to current disability service standards.
  • Provide an information service to families, carers, staff and other government and non-government agencies, particularly with regard to positive behaviour support and skill development issues for clients with a disability who have moderate to complex support needs.
  • Develop, implement and review training programs for families, carers, staff and other government and non-government agencies.
  • Participate in training and professional development activities to maintain and develop competencies to ensure that service delivery is of high quality and consistent with current trends and departmental objectives with a focus on continuous improvement.
  • Maintain client records consistent with departmental and legislative requirements.
  • Prepare high quality reports and undertake assessments within specified time frames and in line with relevant practice and procedures.
  • Actively identify opportunities for improving local client service.
  • Provide mentoring to others in the development of their practice.
