Open Minds is an expanding and innovative not-for-profit organisation providing community based psycho social support services for people with a mental illness, disability or acquired brain injury.
The Position - Works under minimal supervision and involves assisting people with daily living skills and community inclusion and participation activities. Work is undertaken in the context of supporting the person to maintain their well being, explore opportunities and work towards agreed outcomes that are important to, and for, the person.
PHaMs workers provide practical assistance to people with severe mental illness to help them achieve their personal goals, develop better relationships with family and friends, and manage their everyday tasks. One-to-one and ongoing support ensures the individual needs of the PHaMs participants can be addressed. They are assisted to access services and participate economically and socially in the community, increasing their opportunities for recovery
Key Accountabilities include:
- Relationship building
- Service Delivery
- Planning and review
- Reporting, documentation and administration
All Employees:
- Compliance with legislative requirements, OM policy and procedures, confidentiality requirements
- Human rights based approach
- Ability to work collaboratively
- Possess strong written and verbal communication skills.
Must have :
- Current drivers licence and own reliable vehicle
- Cert III in relevant studies OR equivalent knowledge and experience
- Positive Criminal History Screen Clearance
- Blue card - or receipt of application
- Yellow card ( Disability services positive exemption notice card ( OR be willing to obtain)
