Goldfields Mental Health Portal

Full Time and renewed Funding! Initial 12 Month Contract with the possibility of a further 2 years. 

Centrecare in partnership with Goldfields Women's Health Care Centre have been delivering primary mental health services - Goldfields Mental Health Portal in the Goldfields region since 1 July 2017. Under the Integrated Primary Mental Health Care Model this program will focus and deliver quality mental health and wellbeing services for disadvantaged, vulnerable or hard to reach individuals who have, or at risk of mental health condition throughout the Goldfields region.

Covering the Goldfields region this position will deliver place-based low intensity support and psychological interventions to clients. Working within the WA Primary Health Alliance Stepped Care approach, Case coordinators will match client needs to the level of intervention required.

As a Case coordinator you will endeavour to complete regular outreach services as depicted by the needs of the service, communities and individuals incorporating community workshops, group activities and educational seminars. Low intensity psychological interventions will be performed by Case workers and counsellors, dependant on assessed need.

A Few Specifics 

  • Managing an assigned caseload of clients with severe and complex mental health conditions and for assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs.
  • Conduct assessments and complete safety and recovery plans with clients and allocated workers as required;
  • Assess, develop and implement professional support/development strategies and provide expertise for case workers ensuring they can efficiently and effectively manage their case work.
  • Support the Team Leader in developing, planning and evaluating services and in the day to day operations of the service
  • Provide back up support for case workers when necessary.

Mandatory requirements:

  • Qualifications in Psychology, Occupational Therapy, Social Work, Mental Health Nursing or Aboriginal Health Work
  • APHRA or professional body registration
  • Skills and experience in using evidence based focused psychological strategies such as CBT
  • At least 2-5 years post graduate experience
  • Remote post graduate experience highly desirable.
