The Program
PHaMs is about transitioning people to the NDIS and helping people with mental ill-health to live independently and productively through a holistic strengths-based recovery approach.
The Position
Our PHaMs/NDIS case workers walk alongside people with mental health issues who are severely socially isolated. They support a person’s transition to the NDIS, providing advocacy support and referral
They work on what is known as a strength-based recovery program, which focuses on a person's strengths and recognises that a person can live a satisfying and contributing life within the limitations of their illness.

About you:
Selection Criteria
Essential:
- Case management experience particularly working with individuals who experience mental illness, disability or people facing disadvantage or adversity;
- Demonstrated understanding of the issues confronting the disadvantaged, particularly those with mental health issues and/or disability
- Ability to navigate NDIS eligibility and to liaise with Local Area Coordinators, medical professionals and a person’s circle of supports to assist their successful application for NDIS funding.
- Understand the concepts of recovery oriented and strengths-based practice and how this relates to the operation of the PHaMs program.
- Effectively support individuals form a diverse range of backgrounds including people from culturally and linguistically diverse backgrounds, Indigenous Australians, individuals who are homeless or at risk of homelessness and individuals with substance abuse issues;
- Ability to work independently and as a part of a team;
- Ability to demonstrate resilience and commitment.
Desirable:
- Qualifications in Community Services or related discipline will be highly regarded.
- Working knowledge of the NDIS
Other requirements:
- Police Check
- NSW Working with Children Check Clearance
- Current NSW Drivers Licence
