About us

The Disability Trust is offering an exciting career opportunity for the right person to become part of a newly created team. In your role as a NDIS Support Coordinator, you will be responsible for connecting people who have a National Disability Insurance Scheme (NDIS) Support Plan to access a range of supports. This will align with the support items in their NDIS plan which suit their individual needs, across one or more providers.

Key responsibilities of the role:

As an NDIS Support Coordinator you will deliver high quality transition support for young people and adults, living with varying circumstances and levels of complexity. You will match participants with community based and/or mainstream programs to fulfil their individual needs as per individual NDIS plans, whilst providing continuous support and assistance across the time-frame of each plan.

The successful candidate will preferably have current experience working in a similar NDIS role.

Essential Criteria:

  • Relevant tertiary qualification and/ or extensive experience in case management, disability or community services
  • Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and the disability sector
  • Genuine desire to provide the best quality care for people with disabilities
  • Well-developed communication and interpersonal skills including the ability to work and communicate effectively with participants, their families, community agencies and other professionals
  • Ability to work in a very busy environment and to work both independently and as a part of a team
  • Valid Australian Driver’s Licence
  • A cleared Working with Children Check (WWCC)
  • Current National Criminal History Check (cleared within the last 12 months)
