About the Role

We currently have a vacancy for a Home and Community Care Coordinator at our Aged Care Plus Centre in Redfern. The position is responsible for Coordinating services for our ageing population who reside in their own homes in the community.  

In the role of Home Care Coordinator, you will be responsible for undertaking assessments, developing person centred care plans including the review of these plans. You will have a small team of Care Workers you will coach and mentor and the rostering of services for our clients.  Reporting to and working closely with the Operations Manager, this position is an oppoprtunity to demonstrate and develop your people management skills in a varied Home and Community Care role.  The position will also hold accountability for, and consideration of the budgetary, continuous improvement, statuary requirements and human resource needs of the organisation.

 

About You

To be successful in this role, ideally you will have:

  • An appreciation of and the ability and willingness to promote, support and model the values and ethos of The Salvation Army
  • Previous experience in a coordination role, ideally in a Home and Community Care setting
  • Understanding of Community Home Support Program and Home Care Packages
  • Rostering skills
  • Ability to build excellent relationships with all stakeholders including clients, families and staff as well as external 
  • Experience managing Care Workers
  • Passionate, resilient and proactive
  • Ability to multi-task, work under pressure and be part of a team
  • Current unrestricted drivers licence
