We have an opportunity for an experienced Customer Coordinator to lead and coordinate customer entry into the service of this NFP Residential Aged Care, Disability and Community Service provider.  The customer coordination team visits customers in their own homes to conduct assessments and provide information. In this position you will coordinate and promote information sessions with customers; assess customer requirements; regularly consult with customers and their significant others to identify and plan support, care and services; refer as required to relevant internal and external services; and update the client management system with the appropriate customer.

Customer Coordination Team Leader Profile:

  • Previous related experience – strong leadership skills
  • Knowledge of the aged care environment and associated issues
  • Support the Customer Coordination Team in their role to actively promote the service to the community, provide information to customers in relation to services, funding and packages, negotiate and complete the client agreement.
  • Contribute to development of marketing and information products for customers, clients and the broader community
  • Develop and maintain the Customer Coordination Team education and training program
  • Over all responsibility for the customer management system and customer documentation
  • Demonstrated skills in functional assessment of vulnerable people who live in their own homes
  • Demonstrated service delivery in a customer focused role

Qualifications & Skills

  • Credentials in Human Services, Health, Allied Health or similar
  • Tertiary Qualifications in Human Services, Health, Allied Health or similar Marketing credentials – highly regarded
  • Proven proficiency in the use and application of Microsoft Office and client management data bases
