Recovery Support Workers support participants in their recovery journey, building independence and providing holistic support, across a range of Services streams including Personal Helpers and Mentors Program and Supports under the NDIS. 

The role of a Recovery Support Worker is focused on providing opportunities, support and services to develop or redevelop skills, build confidence and reconnect with the community. Ensuring that services accessed by participants are coordinated, integrated and complementary other services in the community. They do this by providing direct and personalised assistance through outreach services, assisting referrals, linking with appropriate services, while constantly monitoring and reviewing progress.

From 1 July 2016, PHaMS funding is transitioning to the National Disability Insurance Scheme (NDIS). It is expected the transition to the NDIS will be complete nationally by the 30th June 2019. During the transition process, the focus of the PHaMS program is to support the NDIS rollout. PHaMS Organisations will begin to meet a range of NDIS requirements while ensuring service continuity for existing clients and:

•          Assisting PHaMS participants to test their eligibility for NDIS supports

•          Assist eligible clients to access NDIS supports; and

•          Provide PHaMS supports to NDIS participants during the transition phase

To deliver PHaMS support and assist eligible clients to access NDIS, Centacare SW NSW engages Multidisciplinary Recovery Support Workers from a range of disciplines which represent the diversity of need presented by PHaMS eligible participants and the accompanying range of service responses required.

SKILLS & EXPERIENCE REQUIRED:

Qualifications

  • Minimum Certificate IV Mental Health, Community Services or related discipline

Experience

  • Skills and experience to liaise, collaborate, facilitate and develop linkages with government, relevant community and clinical services, health and other professionals in relation to participant needs.
  • Experience in writing succinct case notes, follow up and documentation while utilising a Client Information Management System and other reporting platforms

Skills & Knowledge

  • Ability to engage clients
  • An understanding of the impact of mental illness in people's lives, their families and carers (particularly people with complex needs and high levels of disadvantage).
  • Ability to work independently and as part of a team
  • Excellent written and verbal communication skills
  • Strong skills in the use of the Microsoft Office suite of products including Outlook, Excel and Word.
  • Sound administrative and time management skills
  • Demonstrated knowledge of strength-based approach
  • Participant recovery focus – an understanding of and belief in participant-focused, strength-based, recovery-oriented models of service.

KEY SELECTION CRITERIA:

  1. Certificate IV in Mental Health, Community Services or other related health field or two to three years experience working in relevant field.
  2. Demonstrated experience of working with and understanding of mental illness.
  3. Demonstrated understanding of and belief in participant-focused, strength-based, recovery-oriented models of service.
  4. Case management experiences with the ability to coordinate services working within a case management or care coordination framework.
  5. Proven ability to work effectively in a team environment.
  6. Highly developed communication and interpersonal skills strongly focused on creating rapport with participants and their families and  effective networks
  7. A current driver's license.
  8. Ability to maintain confidential information at all times.
  9. A sound understanding and commitment to the philosophy of Centacare and a willingness to work according to the Mission, Vision and Values of Centacare
