We are looking for an experienced Registered Nurse to help coordinate our home care packages and NDIS participants.
About the role
In this permanent part time role (32 hours per week with a view to go full-time), you will work closely with the senior aged care and disability services teams to ensure quality services are delivered and coordinated to older people and people with disabilities of all ages across Melbourne and the Barwon region.
Some of the services you will carry out include:
- Providing advice and advocacy that supports the promotion of individual rights and choices for consumers and participants
- Conducting home care assessments and formulating care plans, managing individual budgets, and monitoring and maintaining care in accordance with Home Care Standards
- Assisting consumers and participants to navigate My Aged Care and the NDIS
- Providing effective, responsive, goal focused and outcome based service provision
- Care Planning – developing, reviewing and monitoring care plans
- Assisting in marketing HenderCare services to consumers and participants
Our ideal candidate
- Registered Nurse with current AHPRA registration
- Relevant experience in case management of consumers receiving home care packages
- Relevant experience in disability service provision
- Ability to take initiative and work autonomously
- High level of professionalism working as a team member
- Able to prioritise work effectively
- Strong commitment to providing excellent service to staff, consumers and participants
- Valid Victorian Working With Children Check
- Valid National Police Check
- Valid Victorian Drivers License
