About the Role: 

We are currently looking for a creative and resourceful person to support and empower NDIS participants to utilise their budgets and to achieve their goals. This role will be separate to the other business streams of Community Lifestyle Support to ensure the Support Coordinator presents a range of choices about providers of supports and is responsible for support coordination only. The successful applicant will have experience in supporting people with a disability, a good understanding of the National Disability Insurance Scheme (NDIS) and implement a person/family centred approach. You will have a confident happy nature with the ability to liaise with mainstream community services, disability providers and stakeholders.

You will report to the Chief Executive Officer and provide monthly reports to the Board.

SELECTION CRITERIA:

Essential Requirements

  • Qualifications: Diploma in Community Health Sector Management or equivalent
  • Proven experience in supporting and empowering people with a disability
  • Proven experience in supporting and empowering families who have a child or family member with a disability
  • Demonstrate an understanding of the NDIS and the role of Support Coordination
  • Ability to deliver a person centred approach
  • Demonstrate strong interpersonal and communication skills
  • Ability to work unsupervised
  • Ability to develop relationships with external organisations and community resources
  • Provide Support Coordination in a manner that empowers choice and control
  • Ability to use our online client management system
  • Current open driving license
  • Be eligible for, or hold a current Working with Children Blue Card
  • Be eligible for, or hold a current Positive Notice Yellow Card

 

Highly desirable knowledge and skills

  • Your own registered vehicle
  • Current First Aid Certificate
  • Experience using Microsoft Office
