CatholicCare is looking for a qualified Youth and Family Case Manager, to work within a dynamic team of specialist workers who provide Early Intervention and Prevention Case Management to Youth – aged 12 years to 18 years of age who are at risk of homelessness or are homeless. You will crucially bring with you to this role a demonstrated passion for youth and their empowerment to reach their full potential!

THE PROGRAM-YOUR TEAM
The Reconnect Youth & Family Worker provides Outreach Based Support within a Person-Centred Case Management Framework to young people who are homeless or at risk of homelessness, between the age of 12 – 18 years and their families.
The aim is to assist young people and their families to maintain contact, and where appropriate, to assist young people to remain within the family home. This is achieved through an early intervention approach, collaborative engagement, service coordination, information, education, referral and advocacy.
The ‘service’ operates across the ACT.

SELECTION CRITERIA
The ideal person we are looking for has:
- Relevant tertiary qualifications in a related field and a minimum of 3 years’ experience in the Community sector working with vulnerable and at youth
- Demonstrated understanding of the client group and their needs
- Ability to liaise and advocate with various stakeholders
- Highly developed problem solving skills
- Demonstrated capacity for sound work organisation and effective use of time
- High standard of written and verbal communication skills
- Demonstrated awareness of the relevant principles and practices such as EEO, OH&S, Confidentiality and Duty of Care,
